You must complete this form after placing your order with Pash & Glou. On the form you will specify white or red (sets of three) and upload your photo(s).
Upon completion of the form and receipt of payment, we will begin processing your order.
For orders of 6 or more bottles, we can accept multiple labels and you will have the opportunity to upload separate images.
If an order is placed and the form is not completed within 21 days and after 2 attempts to contact you via phone or email, at our discretion, we will consider the order a donation and the full value will be remitted to MHS.
Please note that this item is not eligible shipping. We offer a 10% case discount (12 bottles or more); use code 'MHSCASE10' at checkout. If you'd like to purchase other items from the online store, please do so as a separate transaction.
Pick Up Information
Fulfillment will take no longer than 6 weeks from payment and completion of the form, we anticipate many orders will be available sooner.
You must pick up your order from Pash & Glou (414 S Main St, ste 105 | Rochester, MI 48307). When your order is ready, you'll receive an email notification.
